as a student athlete!
All team members make a contribution to the team, not just those who start or play a majority of the time. Participation in a team is not solely defined as playing in a game or match. Participation is defined as involvement in practices and workouts, team activities, and contest participation. Team membership is defined as being selected by the coach to participate as a member of the team.
Coaches of a particular sport will determine the selection process, evaluate students and select team members. C-team membership shall compete and participate as a developmental team. Varsity and Junior Varsity team members are determined by tryouts in some sports.

A Medical Emergency Form is to be used in case of injury to any athlete. 2020-21 Athletic Physical Form must be filed annually before the start of their respective sport season.
No student will be allowed to compete without this form. St. Anne Catholic School requires that every athlete, grades 5-8, have a physical every year and will recognize physicals taken from May of one year to the end of the following school year.
As a condition to participation in any athletic activity, athlete and parent(s)/legal guardian(s) must understand the risks involved in athletic activities. It is not designed to deny the rights of an injured athlete. Statement of Caution Form is designed to provide the school with the degree of protection.
All children are to have their own individual seat belts and must be belted in their seats. All drivers are to have proof of insurance and a copy of a valid Driver’s License and a Sports Transportation Information Form must be on file in the school office. St. Anne Catholic School does not provide transportation but relies on team parents to transport their own child or allow another team parent to transport their child to and from games.
Although normal wear is expected during the use of the uniform, damage to the uniform will be the responsibility of the athlete and parent to whom the uniform is assigned. Athletic Uniform Use Agreement is an agreement between the athlete, parent, and school. The Athletic Uniform is the property of St. Anne Catholic School and is being lent for use during the season of play.
Athlete Responsibilities Form must be on file in order for a student to be eligible to participate in a contest.
St. Anne Catholic School charges each athlete a fee for each sport. This fee is used to defray the cost of Officials, Tournament Fees, Equipment and Court/Field Maintenance and New Uniforms. If your son/daughter has participated in a sport and all forms are on file, an authorization form will need to be filled out and sent to the office for FACTS billing.
